1. Passport application form
2. ID card
3. Fees 1073 Baht(32-page)/1573 (64-page)

Photo will be taken on application, same as ID card updates... doesn't take long
Can be sent on by post, or collected in 3 days.
Application form can be downloaded, saves time having it completed before you go
