Is this form given upon arrival at airport in Bangkok or when you attend an Immigration Office sauy for Retirement Visa renewal?

Foreign National Information
This form shall be used for making record of information of every alien entering and staying in the Kingdom of
Thailand and shall be submit with Notification of Residence for Aliens (section 37 , 38 of Immigration Act, B.E. 2522), or
90-days Notification, or Extension of Stays (all purposes), or Re-Entry Permit, or in all cases involving alien labors.
(If any item does not apply, please write “
-”)

Read the highlighted text.
It states only when applying 90 days extension , Visa extension etc.
Can you imagine the queues at immigration if everyone had to fill one in.

Not as bad as one thinks however I am not really looking forward to filling it in 4 times a year , as they it appears they want us to fill it in at every 90 day extension, so I doubt if any of the information will change from one year to the next.
I sincerely hope once filled that is that but knowing the Thai mindset, I doubt it.

Ask them to photocopy the initial one and then copy every time presented

Take a bag of chicken noodle soup for the photcopy page.

To date no one I know has filled out this form, maybe a storm in a teacup or more likely they got fed up with the minutae on the form and just cannot be bothered to utilise it. Either way in truth they had all the important information anyway apart from bank details and there is no reason why they would want that.
As for where we go that is just to stupid to even ask . But TIT what !!!

As i understand it, it is in the process of being rolled out to all offices. Now in use at many offices. But most fill out only the minimum ....
