Document for title transfer

Document for title transfer

I believe there is a document available from DMV that states "name" receives title to the vehicle when vehicle is paid off while the title remains in the "owners" hands until then. Can anybody help me ID this document?

Sgt, I'm not sure if this is what you are looking for, but I used this form when I transferred my car from my name to my wife's name. The second form is a Power of Attorney in case the owner cannot be present to make the transfer. Both of these forms are from the Dept. of Land Transportation in Udon.
