Retirement Visa Application

I read somewhere on the forum about a new/updated application for a retirement extension, however I’m unable to find it now.
Could someone please direct me to the thread, where It is located?
I do remember, it was not in the Visa section. Also, someone wrote the change was barely noticeable.

last November, the little midget in Chiang Mai saved a new form at me, but did not make me till it out. It seemed like a different font was the only change. They did have TM7 in the tray on the left. I saw them as I was looking for a 90 day report form, which they don't use here.

None of the PDF downloads function

That page has some PDF documents but not the full range of TM numbers

While symbol next to form name on 1st link is for pdf, the file is actually a .doc file, on the 2 I looked at, which needs to be saved and open with microsoft word or other compatable software
