Need a

Medical Certificate
Pass Port
House Book
Passport

I noted they were concerned that I had a Tourist Visa I don't so I don't really know what that was all about.

I went to the one on the Khan Kean Hwy

I watched a film Thai with English subtitles, it had more to do with being courteous then driving rules.

Got my photos thee two licenses one for a car the other for a bike. Just a bit over 900 Baht

The entire thing took about two hours I may have been lucky with timing I don't know

The check you for color blindness, no written test at all.

got mine renewed at the AEK udon Land Transport Office before Christmas.. Old License, Copy of Yellow Book, Copy of Passport, Copy of Retirement Visa Extension.. They didn't ask me for a health certificate... Then a color blindness test, the obligatory safety film, then filed out and got my new license.. Word to the wise at the AEK Udon office.. The door opens at 10AM.. Be there early so you can be first in the queue at the door.. Go straight to the guy in the back of the office that does licenses, give him all the required documents, sign-in and go back into the video room.. Why try to get first or at the front of the queue? They give your new license out in the order you sign in... I was first so had about a 2 minute wait after the video.. When the wife renewed hers back in November she was nearly last and had to wait over an hour after the video to get her new license.

In some places the RTP have begun to enforce the law that an IDP + non Thai license is only acceptable for 3 months after entering Thailand, and that if you're in Thailand for more than that you have to get a local license.

That has no relationship to the validity of the IDP itself.

Luckily the penalty for not having a valid license here is a small fine, sometimes direct payment is acceptable. 8-[

IDP only good for 90 days, police do not have a clue, but have an accident and your insurance certainly will know the rules.

Extended my 1 year license to 5 year today. Took about an hour and a half, start to finish.
